Job summary

Prevea Health is seeking a Back Registered Nurse (RN) Oncology Navigator for Breast Surgery in Green Bay, WI. This 32-hour-per-week, benefit-eligible role supports an oncology navigation program and coordinates education and resources for diverse patient populations.

You will work with internal and external departments to ensure smooth transitions of care, maintain patient education, and help meet quality and patient-satisfaction metrics under general administrative direction.

Qualifications

• Graduate of an accredited nursing program (Required).
• Bachelor's Degree in Nursing (BSN) preferred.

Responsibilities

• Support and advance the oncology navigation program goals and workflows.
• Assess barriers to care and connect patients with community resources.
• Coordinate care transitions between Prevea, HSHS, and oncology partners.
• Provide patient education and nursing judgment-related guidance.
• Attend tumor boards as patient advocate and participate in survivorship activities.
• Perform telephone triage and triage patients to appropriate settings.
