Qualifications

• Active nursing license in good standing is REQUIRED for this role.

• Ability to remain calm in overnight clinical situations.

Responsibilities

• Initiate patient outreach at the beginning of the care episode.
• Serve as primary clinical contact for patients throughout their episode of care.
• Respond to patient concerns or symptoms outside normal office hours.
• Conduct proactive outreach calls to monitor patient progress.
• Document all patient interactions within the care management platform.
