Job summary

LiveWell Homecare Agency seeks an RN Nurse Navigator Triage to perform nursing triage and health advice via phone and in person, assessing patient acuity and determining medical needs. You will ensure continuity of care by delivering results, education, and protocol-driven clinical guidance with timely communication and documentation.

The role operates in a clinical setting with flexible shifts, including days and nights, and travel for training or meetings.

Qualifications

• Graduate from a specialty nursing program is required; BSN is preferred.
• Five years of nursing experience, especially in ICU, ED, or critical care.
• Proficiency with computer systems and Microsoft Office is essential.
• Certification in specialty areas is advantageous.

Responsibilities

• Perform patient assessments, prioritize urgent calls, and coordinate care across departments.
• Educate patients and families, schedule appointments, and support staff training.
• Facilitate communication between patients, providers, and other healthcare entities with proper documentation.
