Job summary

Geisinger is seeking an RN Nurse Navigator Triage to provide nursing triage services by phone and in person where applicable. The role assesses acuity, delivers protocol-driven guidance, and coordinates care with other departments to ensure timely treatment and patient safety.

The position requires a BSN (preferred) and extensive nursing experience, with ICU/ED exposure preferred. Strong computer skills and proficiency with Microsoft Word, Excel, PowerPoint, SharePoint, Windows, and Teams are

Qualifications

• Performs patient assessment via phone and documents symptoms per protocol.
• Initiates and coordinates emergency treatment when needed.
• Prioritizes high-priority calls addressing urgent needs.
• Provides education to patients and families.
• Coordinates care across departments, specialties and sites.
• Assists orientation of new personnel.
• Manages ongoing patient communication and provider notifications.
• Explains medical conditions to patients.
• Ensures timely patient documentation.
• Collaborates with external organizations including insurance, home health, and other facilities.

Responsibilities

• Performs patient assessment via phone and documents patient chief complaint, symptoms and history using established protocols.
• Initiates and coordinates emergency treatment when necessary.
• Prioritizes incoming high priority calls addressing urgent patient needs while reporting information to triage provider.
• Provides education to patients and families.
• Schedules or facilitates scheduling of emergency appointments.
• Coordinates patient care and requests across departments, specialties and sites.
• Assists with orientation of new personnel to ensure appropriate training.
• Executes and documents ongoing patient and customer communication and responds promptly to needs.
• Reviews and reinforces instructions to ensure understanding and compliance with treatment plan.
• Explains medical conditions to patients.
• Manages patient concerns following clinical protocols with service recovery focus.
• Ensures patient documentation is completed and routed timely.
• Suggests improvements and offers solutions.
• Assists in coordination of care with external organizations.
