Chapters Health System is seeking an RN for nights and weekends to perform after-hours field visits that support patients, families, and community partners. You will build strong relationships with care teams, referral sources, and patients while delivering compassionate nursing care.

The role requires home visits across assigned territories, with emphasis on assessment, care planning, and coordination with interdisciplinary teams.

Qualificações

• Current RN license in Florida.
• Minimum 1 year nursing experience; hospice/hospital preferred.
• Experience using EMR/EHR systems.
• English communication required; Spanish bilingual preferred.
• Valid driver’s license and auto insurance.

Responsabilidades

• Makes after-hours visits to patients in home, facility, or hospital.
• Assesss patient needs and plan appropriate nursing interventions.
• Provide direct care as part of IDG with psychosocial and physical care.
• Document care per policy; ensure continuity and coordination.
• Prioritize, collaborate with other professionals, and educate patients and families.
