Qualifications

• Graduate of an accredited nursing school.
• Connecticut RN license required; BLS certification.
• Excellent assessment and critical thinking skills.

Responsibilities

• Implements and monitors patient care plans and communicates patient condition.
• Acts as primary coordinator of multidisciplinary patient care.
• Executes physician and nursing orders and documents care.
• Coordinates discharge planning with the healthcare team.
• Assess patient needs across physical, psychosocial, cultural, spiritual and age-related domains.
• Participates in daily interdisciplinary patient rounds and supports outcomes.
