Mercy is seeking a Nurse Navigator to lead care coordination for inpatient and virtual care programs, guiding patients and families through complex care journeys.

This hybrid role supports Mercy Virtual Care Center with pre-op education, discharge planning, post-discharge follow-up, and collaboration with physicians, nurses, and behavioral health teams to ensure safe, high-quality care.

Qualifikationen

• Graduate of an accredited School of Nursing.
• Current RN license in the applicable state.
• Minimum of 2-3 years of nursing experience.
• Basic Life Support (BLS) certification.

Aufgaben

• Coordinate and oversee patient care for inpatient and outpatient populations.
• Assess patient needs and make recommendations based on illness severity and care requirements.
• Develop, implement, and evaluate individualized care plans with interdisciplinary teams.
• Conduct patient rounds and serve as a primary resource for patients and families.
• Provide pre-operative education, discharge planning, and follow-up care.
• Ensure patients understand treatment plans and discharge instructions.
