Job summary

Mercy Nurse Navigator (Part-Time, Remote) guides patients through their healthcare journey, coordinating care from pre-treatment through follow-up. The role partners with physicians, bedside nurses, and interdisciplinary teams to ensure timely, safe, high-quality care and to connect patients with the resources they need.

The position emphasizes patient education, discharge planning, utilization management, and quality improvement initiatives.

Qualifications

• Current RN license in the state of practice.
• Minimum of 2-3 years of nursing experience.
• BLS certification.

Responsibilities

• Coordinate care for assigned inpatient and outpatient populations.
• Guide patients through their treatment journey and serve as a primary resource.
• Provide patient and family education, including pre-operative preparation and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes, patient satisfaction, and continuity of care.
• Assist with LOS management and utilization review activities.
• Participate in quality improvement initiatives and implementation of best practices.
• Serve as a clinical resource and mentor to members of the care team.
• Maintain accurate documentation within the electronic medical record (EPIC).
