Job summary

SAN-Jose-Healthcare- seeks a Medicare/MDS Coordinator to lead the IDT in MDS assessments and ensure timely completion according to State and Federal regulations. The role reports to the Director of Nursing Services and focuses on coordinating assessments, plans of care, and necessary treatments while maintaining high standards of nursing care and documentation.

Qualifications

• RN licensure required in nursing.
• English communication skills required for business operations.
• Administrative and organizational abilities needed for coordination tasks.
• CPR certification preferred.
• Two years of nursing experience in long-term care preferred.
• Supervisory experience preferred.

Responsibilities

• Coordinate Medicare/MDS resident assessment process.
• Ensure IDT completes MDS assessments in a timely manner.
• Coordinate development, implementation and evaluation of care plans.
• Coordinate and perform treatments, medications or nursing interventions as ordered.
• Provide nursing care adhering to infection control standards.
• Follow safety policies in nursing care.
• Initiate emergency measures per center policy and nursing standards.
• Maintain essential documentation per Federal and State regulations and company policy.
• Participate in meetings and Inservices as assigned.
• Present a professional image to consumers and resolve concerns.
