Job summary

Heritage Manor Care in Escondido, CA seeks a Medicare/MDS Coordinator under the direction of the Director of Nursing Services. You will coordinate the MDS resident assessment process with the Interdisciplinary Team to ensure timely completion in compliance with state and federal regulations.

You will implement and evaluate the plan of care, perform required treatments and nursing interventions, and maintain infection control standards while following safety policies and center procedures.

Qualifications

• Current RN License in state of California.
• 2 years experience as an MDS Coordinator in a Skilled Nursing Facility.
• Background check with no history of abuse.

Responsibilities

• Coordinate the Medicare/MDS resident assessment process.
• Ensure the Interdisciplinary Team completes the MDS Assessment in a timely manner.
• Coordinate development, implementation and evaluation of plan of care.
• Coordinate and perform, administers or implements as needed treatments, medications or other nursing interventions as indicated by the resident plan of care or as ordered by the physician.
• Coordinate and provide as needed nursing care in accordance with infection control standards.
• Follow safety policies in performing nursing care.
• Coordinate and initiates as needed emergency measures according to center policy and within standards of nursing practice.
