Job summary

PowerBack Rehabilitation in Reading, PA is seeking an experienced MDS Coordinator - RN to ensure timely completion of the MDS assessment and accurate care planning in a long-term care setting. You will coordinate MDS reviews, collaborate with nursing, dietary, social services and physicians to optimize reimbursement and patient care while maintaining compliance with high standards of care.

The role requires RN licensure, at least one year in long-term care, and a commitment to quality, teamwork

Qualifications

• Must be a graduate of an accredited School of Nursing with current RN licensure.
• At least one year of long-term care nursing experience is required.

Responsibilities

• Manage the MDS process and ensure timely completion of the MDS assessment tool.
• Review care plans and delivery for physical, cognitive, and social factors to meet high standards of care.
• Coordinate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement.
