PowerBack Rehabilitation in Gardendale, AL is seeking a Clinical MDS Coordinator (RN) to ensure timely and accurate completion of the MDS assessment tool and proper care plan review for residents. The role collaborates with nursing, dietary, social services, restorative and physician teams to optimize reimbursement and care quality.

RN licensure and LTC experience are required; LPNs with MDS experience may be considered.

Qualifications

• Graduate of an accredited School of Nursing with current RN licensure.
• LPNs with MDS experience may be considered.

Responsibilities

• Complete MDS assessment tool timely and accurately.
• Review care plans and delivery for physical, cognitive, and social factors to ensure high standard of care.
• Manage Medicare/Medicaid case-mix documents for appropriate reimbursement.
• Conduct concurrent MDS reviews to maximize RUG categories.
• Integrate information from nursing, dietary, social services, restorative, rehabilitation and physician services for reimbursement.
