The position supports quality outcomes, with a focus on timely data and ongoing process improvement. RN license in Alabama is preferred and prior SNF MDS experience is highly valued.

Qualifications

• Active Alabama RN license required or preferred.
• Experience as MDS Coordinator in a Skilled Nursing Facility (SNF) preferred.
• Knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and PDPM.
• Experience with CMS/Medicare/Medicaid regulations and reimbursement processes.

Responsibilities

• Coordinate and complete MDS assessments in accordance with CMS, Medicare, Medicaid, and state regulations.
• Ensure timely and accurate completion of MDS, CAA, and care plan processes.
• Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
• Monitor reimbursement and case mix opportunities.
• Review documentation to ensure accuracy and regulatory compliance.
• Participate in Medicare meetings and utilization reviews.
• Assist with survey preparedness and regulatory compliance initiatives.
• Analyze quality measures and support performance improvement efforts.
• Maintain knowledge of current MDS guidelines and reimbursement practices.
