Job summary

Northgate Health & Rehab is seeking an experienced MDS Coordinator (RN) in Bessemer, AL to join our clinical leadership team. The role emphasizes accurate MDS assessments, regulatory compliance, and reimbursement optimization within a Skilled Nursing Facility.

The MDS Coordinator will ensure timely MDS, CAA, and care plan processes, collaborate across disciplines, and support survey readiness and performance improvement initiatives. Alabama RN licensure is preferred for this position.

Qualifications

• Active Alabama RN license preferred.
• Experience as MDS Coordinator in a Skilled Nursing Facility (SNF) preferred.
• Knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and PDPM.
• Strong assessment, analytical, and documentation skills.
• Experience with electronic medical records and MDS software.

Responsibilities

• Coordinate and complete MDS assessments in accordance with CMS, Medicare, Medicaid, and state regulations
• Ensure timely and accurate completion of MDS, CAA, and care plan processes
• Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members
• Monitor reimbursement and case mix opportunities
• Review documentation to ensure accuracy and regulatory compliance
• Participate in Medicare meetings and utilization reviews
• Assist with survey preparedness and regulatory compliance initiatives
• Analyze quality measures and support performance improvement efforts
• Maintain knowledge of current MDS guidelines and reimbursement practices
