The MDS Nurse at Ely Manor within Ciena Healthcare is responsible for assisting with accurate and timely completion of the Resident Assessment Instrument (RAI), including MDS, CAAs, and care planning, while ensuring supporting documentation meets federal and state regulations. This role supports individualized care planning and reflects residents' clinical status.

Qualifications

• RN licensure and active eligibility to practice.
• CPR certification (current or within 90 days).
• Experience with MDS/RAI and assessment processes preferred.
• Knowledge of RAI, PDPM, ICD-10 coding and care planning.

Responsabilités

• Assist with accurate and timely completion of MDS/RAI data.
• Support CAAs, care planning and documentation.
• Ensure assessments reflect residents' clinical status.
