Job summary

Ely Manor in Michigan is seeking an MDS Nurse to support accurate RAI completion, MDS/CAA, care planning, and timely documentation in accordance with federal and state regulations. This role collaborates with the IDT to ensure residents receive individualized care plans that reflect their clinical status.

Qualifications

• Prior MDS/RAI experience strongly preferred.
• Knowledge of RAI, PDPM, ICD-10, Quality Measures, and Care Area Assessments and Care Planning processes.
• Knowledge of regulatory standards and compliance requirements.
• Demonstrated ability in independent functioning and strong leadership.

Responsibilities

• Coordinates scheduling the RAI process with the interdisciplinary team.
• Completes the MDS, CAA’s and care plans within regulatory timeframes.
• Coordinate with interdisciplinary team (IDT) members to gather timely, accurate assessment data to ensure timely completion of the MDS.
• Ensure compliance with state and federal guidelines as it relates to the PDPM and Case Mix payment systems.
• Communicates with the interdisciplinary team regarding changes in resident care needs.
• Coordinates, identifies, and/or initiates significant change MDS’
• Conducts a thorough resident assessment during quarterly reviews by interviewing the residents, staff, and family.
• Coordinates and conducts PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS log for monitoring of Medicare days in the absence of the MDS Coordinator.
• Occasionally participates in direct and indirect Resident care activities as directed by supervisor.
• In conjunction with the Social Worker, coordinates Resident Care Conferences to ensure residents and/or responsible parties participate in resident care planning for the assigned resident.
• Ensure compliance with state and federal legal, regulatory, accreditation and reimbursement guidelines.
• Attends and participates in departmental/facility meetings, as required.
• Adheres to all safety and risk management protocols and programs.
• Participates in Quality Assurance and Performance Improvement initiatives.
• Interprets Company policy and makes decisions based on the application of those policies where applicable.
