Ciena Healthcare's The Manor of Novi is seeking an MDS Nurse responsible for accurate and timely completion of the RAI, MDS, CAAs, and care planning in accordance with federal and state regulations. This role ensures assessments reflect each resident’s clinical status and supports individualized care planning.

Qualifications

• Prior MDS/RAI experience strongly preferred.
• Knowledge of RAI guidelines, PDPM, ICD-10 coding, Quality Measures, and Care Area Assessments and Care Planning processes.
• Knowledge of regulatory standards and compliance requirements.
• Demonstrated ability in independent functioning and strong leadership.

Responsibilities

• Complete MDS/RAI accurately and timely to support care planning.
• Ensure CAAs and related documentation comply with regulations.
• Support individualized resident care planning.
