Job summary

Ely Manor in Allegan, MI is hiring an MDS Nurse to ensure accurate and timely completion of the MDS, CAAs, and care plans while coordinating with the interdisciplinary team. The role emphasizes regulatory compliance and PDPM alignment to support resident-centered care.

The ideal candidate has RN licensure with CPR certification, experience in MDS/RAI, and strong leadership abilities to guide care planning and documentation for residents under federal and state guidelines.

Qualifications

• Prior MDS/RAI experience strongly preferred.
• Knowledge of RAI, PDPM, ICD-10, Quality Measures, and Care Planning.
• Knowledge of PDPM and regulatory requirements.
• Strong leadership and independent functioning.

Responsibilities

• Coordinate scheduling the RAI process with the interdisciplinary team.
• Complete the MDS, CAAs, and care plans within regulatory timeframes.
• Gather timely assessment data to ensure MDS completion.
• Ensure compliance with PDPM and Case Mix payment systems.
• Communicate changes in resident care needs to the IDT.
• Participate in PPS meetings and maintain PPS log.
