Job summary

North Shore Healthcare in Wisconsin seeks an experienced RN leader to direct the Resident Assessment Instrument (RAI) process, ensuring timely and accurate resident assessments in line with Medicare, Medicaid, OBRA, and other payor requirements.

You will oversee PPS assessments and ensure documentation supports services and reimbursement, while reflecting residents' physical, mental, and psychosocial status across the care team.

Qualifications

• Registered Nurse in good standing with state board.
• Geriatrics experience and PPS/OBRA knowledge.
• Excellent clinical assessment skills and documentation.
• Strong organization and independent decision-making.
• Familiarity with state and federal regulations governing care centers.

Responsibilities

• Direct the RAI process to ensure timely, accurate assessments.
• Ensure RAI reflects resident status and supports optimal reimbursement.
• Maintain appropriate documentation for services and assessments.
• Collaborate with clinical teams to ensure quality care and compliance.
