Job summary

Northern Nevada State Veterans Home is seeking an MDS Coordinator to lead the MDS assessments and care planning, ensuring compliance with federal, state, and community guidelines. You will coordinate with interdisciplinary teams to drive accuracy in documentation and reimbursement, contributing to high-quality resident care.

Qualifications

• Current RN license in the state of employment required.
• RAC-CT certification preferred or willingness to obtain within designated timeframe.
• Prior experience in MDS coordination within a skilled nursing setting required.
• Strong knowledge of RAI process, PDPM, Medicare guidelines, and care planning.
• Excellent critical thinking, time management, and communication skills.

Responsibilities

• Complete timely and accurate MDS assessments in compliance with federal, state, and community guidelines.
• Ensure thorough, collaborative interdisciplinary care planning for each resident.
• Maintain detailed documentation that reflects resident care, progress, and reimbursement accuracy.
