HMR Veterans Services in the United States seeks a detail-oriented Registered Nurse with MDS experience to coordinate and complete comprehensive resident assessments. Qualifications

• Nursing degree from an accredited college or university.
• Current, active RN license in the state of practice.
• Minimum of one year of experience in a hospital, long-term care facility, or related healthcare setting.
• Experience completing and coordinating MDS assessments in a long-term care or skilled nursing facility.
• Strong organizational, analytical, and communication skills.
• Knowledge of federal and state regulations related to resident assessments and care planning.

Responsibilities

• Conduct and coordinate the completion of resident assessments (MDS) in accordance with federal and state requirements.
• Ensure the timely and accurate completion of MDS, CAAs, and care plans.
• Collaborate with interdisciplinary team members to develop and maintain individualized resident care plans.
• Monitor assessment schedules and ensure compliance with Medicare, Medicaid, and regulatory guidelines.
• Review clinical documentation to support accurate coding and reimbursement.
• Assist with surveys, audits, and quality improvement initiatives.
• Serve as a clinical resource for nursing staff regarding assessment and documentation requirements.
• Maintain current knowledge of MDS regulations and reimbursement processes.
