Monarch Healthcare Management is seeking a skilled MDS Coordinator (RN) to oversee the MDS assessment lifecycle, ensuring timely completion with accuracy and regulatory compliance across our Minnesota facilities. You will collaborate with the Interdisciplinary Team (IDT) to coordinate care plans, support nursing staff, and maximize case mix while upholding quality of care.

Qualifications

• Must be a Registered nurse (RN) in Minnesota.
• Must have CPR or emergency care certification for health care providers.
• Ability to communicate effectively in writing and verbally while directing RAI.
• Able to perform essential functions with or without accommodation.
• Pass background check per Minnesota DHS guidelines.
• Healthcare facility experience preferred.
• Experience with MDS and RAI preferred.

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Ensure timely MDS completion with accuracy and regulatory compliance.
• Coordinate care plan completion with the Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff for accurate assessments and care plans.
