Qualifications

• Must be a Registered nurse (RN) in Minnesota.
• Current CPR certification or willingness to obtain.
• Strong verbal and written communication for RAI processes.
• Ability to coordinate care plans with the IDT.
• Experience with MDS and RAI preferred.

Responsibilities

• Oversee completion, editing, locking and transmission of MDS assessments.
• Coordinate care plan completion with the Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff to ensure accurate assessments and quality care.
