Hudson Elms Nursing & Rehab is seeking an experienced RN MDS Coordinator to join our team in Ohio. The role focuses on completing MDS 3.0 assessments, developing care plans, and ensuring accurate documentation for regulatory compliance.

The ideal candidate holds an RN license, has MDS/RAI experience in long-term care, understands Medicare/Medicaid guidelines, and demonstrates strong organizational and communication skills. RAC-CT certification is preferred or obtainable after hire.

Qualifications

• RN license in good standing.
• Experience with MDS 3.0, care planning, and RAI processes in a long-term care setting.
• Knowledge of Medicare/Medicaid regulations and reimbursement guidelines.
• Excellent organization, communication, and computer skills.
• RAC-CT certification preferred (or willingness to obtain upon hire).

Responsibilities

• Complete and coordinate accurate and timely MDS 3.0 assessments in compliance with federal and state regulations.
• Develop and update individualized care plans in collaboration with the interdisciplinary team.
• Ensure appropriate documentation supports coding and reimbursement accuracy.
• Submit MDS assessments and track CMS submissions to ensure error-free processing.
• Participate in care planning meetings, QA audits, and survey readiness.
• Educate and support clinical staff in understanding MDS and RAI processes.
