Job summary

Skilled Nursing Facility seeks an MDS Coordinator who must be a nurse (RN or LPN). You will oversee Medicare/Medicaid case-mix documentation, conduct MDS reviews, and coordinate PPS submissions to ensure appropriate reimbursement.

You will integrate nursing, dietary, social recreation and physician services, schedule resident conferences, and manage data entry and regulatory timeframes for MDS assessments. Strong English communication and LT care experience preferred; current, unencumbered state

Qualifications

• Registered Nurse with current unencumbered state licensure.
• Long Term Care experience preferred.
• Ability to read, write, speak and understand English.

Responsibilities

• Oversees completion of Medicare/Medicaid MDS and PPS documentation.
• Conducts regular MDS reviews to optimize RUG categories and reimbursement.
• Coordinates interdisciplinary participation for MDS for new admissions and ongoing assessments.
• Schedules resident care conferences and ensures regulatory timeframes are met.
• Manages data entry and electronic submission of MDS assessments.
