Batavia Rehab and Healthcare Center is seeking an experienced MDS Coordinator (RN) to manage the RAI process from admission to discharge, ensuring accuracy and compliance with federal, state, and certification guidelines.

The ideal candidate holds an RN license, has MDS experience, and is passionate about senior care. This role requires teamwork with the Director of Nursing and provides competitive pay and benefits.

Qualifications

• Currently licensed as an RN in the practicing state.
• Prior MDS experience preferred.
• Experience in LTC/SNF/AL/MC settings preferred.
• Knowledge of Case-Mix and Medicare PPS processes.
• Understanding OBRA and MDS requirements.
• Familiar with care planning processes.
• Strong written and verbal communication.

Responsibilities

• Ensure timely, accurate completion of the RAI and care management processes from admission to discharge.
• Coordinate information systems operations and education for the clinical department as needed.
