Job summary

Life Care Centers of America in Fort Wayne, IN is seeking an RN MDS Coordinator to oversee accurate and timely MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual regulations and Life Care standards. The role requires an active RN license, a nursing degree (Associate’s or Bachelor’s), and at least two years of nursing experience (geriatric experience preferred).

CPR certification is required or obtained during orientation, with ongoing training and adherence to company

Qualifications

• Two (2) years of nursing experience, geriatric nursing preferred.
• Currently licensed as an RN with an active license in good standing.
• CPR certification obtained upon hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition identified by the MDS to the DON.
• Educate direct care staff on CMS RAI updates and Nursing Facility regulations.
• Review Interdisciplinary Comprehensive Care Plan and Final Validation Reports for CMS QIES submission.
• Attend PPS meetings and other scheduled meetings; perform staff nurse duties as required.
• Maintain confidentiality and adhere to Life Care’s policies and Code of Conduct.
