Job summary

The RN MDS Coordinator at Life Care Centers of America coordinates and assists with completion and submission of MDS assessments, CAAs, and Care Plans in accordance with CMS RAI Manual Regulations and Life Care standards.

The position requires an active RN license, 2 years of nursing experience (geriatric preferred), and CPR certification. You will uphold confidentiality and work within an interdisciplinary team to ensure quality patient care.

Qualifications

• RN licensure in good standing is required.
• Minimum 2 years of nursing experience; geriatric experience preferred.
• CPR certification is required or obtained during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI updates impacting documentation.
• Review Interdisciplinary Comprehensive Care Plans and CMS reports.
• Attend PPS and other required meetings and support daily operations.
