The ideal candidate holds an active Florida RN license with MDS experience in a SNF setting and strong knowledge of CMS regulations. Excellent communication and organizational skills are essential for success.

Qualifications

• Must hold an active Florida RN license.
• Previous MDS Coordinator experience in a Skilled Nursing Facility is preferred.
• Strong knowledge of RAI, Medicare, and CMS regulations.
• Experience with care planning and interdisciplinary team collaboration.
• Excellent communication, organization, and documentation skills.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state regulations.
• Ensure timely and accurate completion of the Resident Assessment Instrument (RAI) process.
• Collaborate with nursing, therapy, social services, and other departments to develop and update care plans.
• Monitor Medicare and reimbursement documentation requirements.
• Maintain compliance with CMS guidelines and facility policies.
• Participate in quality improvement initiatives and survey readiness activities.
• Educate staff on MDS processes and documentation standards.
