Apex-Healthcare in Hialeah, FL is seeking an experienced MDS Coordinator to join our skilled nursing team. You will oversee MDS assessments, ensure RAI timelines, and collaborate with nursing, therapy, social services, and other departments to develop and update comprehensive care plans.

The role requires Florida nursing license, MDS coordination experience in SNF, and strong knowledge of CMS/RAI regulations.

Qualifications

• Active Florida Nursing license required.
• Previous MDS Coordinator experience in a Skilled Nursing Facility preferred.
• Strong knowledge of RAI, Medicare, and CMS regulations.
• Experience with care planning and interdisciplinary team collaboration.
• Excellent communication, organization, and documentation skills.
• RAC-CT certification is a plus.

Responsabilités

• Coordinate and complete MDS assessments in accordance with federal and state regulations.
• Ensure timely and accurate completion of the Resident Assessment Instrument (RAI) process.
• Collaborate with nursing, therapy, social services, and other departments to develop and update care plans.
• Monitor Medicare and reimbursement documentation requirements.
• Maintain compliance with CMS guidelines and facility policies.
• Participate in quality improvement initiatives and survey readiness activities.
• Educate staff on MDS processes and documentation standards.
