Qualifications

• Licensed Registered Nurse (RN) in Florida.
• Minimum of 2 years’ experience in a skilled nursing facility.
• Strong knowledge of MDS, Medicare regulations, and care planning.

Responsibilities

• Oversee care standards for residents and ensure quality of care.
• Conduct assessments and interviews for new residents to build personalized plans.
• Design and monitor care plans with measurable goals for short- and long-term improvement.
• Facilitate interdisciplinary meetings with health professionals, families, and caregivers.
• Partner with specialists to achieve best resident outcomes.
• Lead nursing management meetings and provide in-service trainings on MDS, RAPS, and care triggers.
• Manage MDS/Care Planning schedules for non-Medicare residents and meet deadlines.
