Job summary

Commonwealth Care of Roanoke is seeking an experienced RN to join as an MDS Coordinator, leading the interdisciplinary team in MDS assessments and care planning. The role focuses on compliance with Medicare/Medicaid and state/federal guidelines, while mentoring LPNs and ensuring timely, accurate documentation.

You will supervise MDS/Care Plan processes, coordinate assessments, and contribute to improved resident outcomes through education and collaborative care planning.

Qualifications

• Active RN license in good standing
• Minimum of two years experience with the MDS process
• RAC-CT certification preferred and ongoing professional development

Responsibilities

• Oversee and mentor LPNs in the MDS Coordinator role and support care planning
• Perform MDS assessments, triggers, and care plans through chart reviews and collaboration
• Coordinate assessments to ensure timely completion compliant with State and Federal guidelines
• Lead the interdisciplinary team to develop resident-specific care plans
• Ensure documentation supports coding decisions and regulatory compliance
• Provide training to the ID team and maintain accurate CMI scores
• Attend clinical meetings to adjust plans based on resident changes
