Complete Care - New Jersey & PA is hiring a Registered Nurse-RN-MDS Coordinator in New Jersey. The role focuses on quality outcomes, accurate assessments, and individualized care plans within a collaborative interdisciplinary team.

You will coordinate MDS assessments, work with nursing, therapy, social services, and others to develop care plans, ensure regulatory compliance and reimbursement accuracy, and educate staff on MDS processes.

Qualifikationen

• Associate's or Bachelor's Degree in Nursing.
• Current, unrestricted New Jersey RN license.
• Long-term care experience preferred.
• RN-MDS Coordinator or MDS assessment experience preferred.
• Strong clinical assessment, communication, and organizational skills.
• Knowledge of federal and state MDS regulations.
• Ability to work collaboratively with interdisciplinary teams.

Aufgaben

• Coordinate and complete accurate MDS assessments per guidelines.
• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as residents' conditions change.
• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
• Work closely with leadership to maintain compliance and promote continuous quality improvement.
• Help ensure Complete Care continues to provide the highest level of resident-centered care.
