Job summary

West Roxbury Health & Rehabilitation Center, part of Elderwood Administrative Services, seeks an RN – MDS Coordinator to connect clinical assessment with individualized resident care. You will lead MDS processes and care planning, collaborating with nursing, therapy, dietary, and medical records teams.

The role emphasizes Medicare/Medicaid knowledge, PDPM awareness, and strong communication, organization, and detail orientation in a busy skilled nursing setting.

Qualifications

• Current RN license in Massachusetts.
• Nursing degree from an accredited college or university.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of the MDS and care planning process.
• Experience with MDS 3.0 scheduling, coding, and submission preferred.
• Experience with PointClickCare preferred.
• Knowledge of PDPM and Case Mix Index (CMI) preferred.
• Experience with Medicare/PPS, Medicaid, Managed Care, and reimbursement preferred.
• Strong communication, organization, and time-management skills.

Responsibilities

• Lead the MDS and Resident Assessment Process for short-term and long-term residents.
• Develop, update, and coordinate individualized care plans.
• Support Medicare, Medicaid and reimbursement processes.
• Promote compliance, quality and accountability across regulations.
