Parkway Health & Rehabilitation Center in Boston, MA is seeking an RN – MDS Coordinator to connect clinical assessment with individualized resident care across short-term and long-term services.

This role leads the MDS and care planning process, ensuring on-time submissions, regulatory compliance, and accurate documentation for Medicare, Medicaid, and Managed Care. Collaboration with an interdisciplinary team is essential to optimize resident outcomes and reimbursement processes.

Qualifications

• Current RN license in Massachusetts and nursing degree from an accredited college or university.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of MDS and care planning.
• Familiarity with Medicare PPS, Medicaid, and reimbursement processes.
• Knowledge of PDPM and Case Mix Index (CMI) preferred.
• Strong communication, organizational, and time-management skills.
• Ability to collaborate with residents, families, and interdisciplinary team.

Responsibilities

• Lead the MDS/PPS and OBRA assessments for residents.
• Establish and maintain schedules for required assessments per regulations.
• Ensure MDS assessments and related documentation are accurate and submitted on time.
• Monitor MDS component completion by other disciplines and address gaps.
• Develop and update individualized care plans based on assessments.
• Conduct care plan conferences with residents, families, and team.
• Coordinate with nursing, rehabilitation, social services, dietary, and medical records.
• Support Medicare, Medicaid, Managed Care, and reimbursement processes.
• Promote compliance and survey readiness; maintain regulatory knowledge.
• Serve as a clinical resource on assessment and planning requirements.
