Job summary

Epic Healthcare in New Jersey is seeking an experienced MDS Coordinator to ensure accurate MDS assessments and regulatory compliance across our long-term care residents.

You will collaborate with interdisciplinary teams, complete timely submissions, participate in care plan meetings, and help maintain pristine clinical documentation. RN license in NJ and MDS knowledge required; certification preferred.

Qualifications

• Active RN license in New Jersey.
• MDS certification preferred, or the willingness to obtain certification within a specified time.
• Previous experience in a long-term care setting or with MDS assessments is preferred.
• Strong understanding of the MDS process, coding, and regulatory requirements.
• Excellent communication and organizational skills.
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Complete and submit MDS assessments for all residents in a timely and accurate manner.
• Collaborate with interdisciplinary teams to create and update care plans that align with residents' needs and preferences.
• Monitor the completion of all assessments and ensure they meet federal and state regulations.
• Assist in conducting audits and responding to regulatory inquiries.
• Participate in care plan meetings and provide input on residents' needs.
• Review and update clinical documentation to ensure accuracy and compliance.
