Job summary

Talentify is seeking an MDS Coordinator in New Jersey to ensure accurate MDS assessments and regulatory compliance. You will collaborate with an interdisciplinary care team to align care plans with residents' needs and preferences.

The role involves reviewing and submitting MDS assessments, and participating in care plan meetings while ensuring documentation meets federal and state requirements. An RN license in NJ is required; MDS certification is preferred.

Qualifications

• Active RN license in New Jersey.
• MDS certification preferred or willingness to obtain within specified time.
• Previous experience in a long-term care setting or with MDS assessments is preferred.
• Strong understanding of the MDS process, coding, and regulatory requirements.
• Excellent communication and organizational skills.
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Complete and submit MDS assessments for all residents in a timely and accurate manner.
• Collaborate with interdisciplinary teams to create and update care plans that align with residents' needs and preferences.
• Monitor the completion of all assessments and ensure they meet federal and state regulations.
• Assist in conducting audits and responding to regulatory inquiries.
• Participate in care plan meetings and provide input on residents' needs.
• Review and update clinical documentation to ensure accuracy and compliance.
