Spring Grove Rehabilitation and Healthcare Center in New Providence, NJ, is seeking a full-time, in-person MDS Coordinator. You will lead MDS assessments to support resident care planning, reimbursement, and regulatory compliance.

Qualifications

• RN license required.
• Minimum of three (3) years of clinical experience in a long-term care setting.
• Prior MDS/RAI experience required.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments in accordance with regulatory requirements and deadlines.
• Verify compliance with federal, state, and facility regulations related to MDS documentation and submission.
• Supervise MDS data entry, validation, and transmission processes to ensure accuracy and timeliness.
• Identify, investigate, and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the Director of Nursing and leadership.
