Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking an experienced MDS Coordinator to join our clinical leadership team. This role focuses on accuracy of MDS assessments, regulatory compliance, and resident-focused care within a collaborative interdisciplinary environment.

The ideal candidate holds an active NJ RN or LPN license, has MDS 3.0/RAI knowledge, and can lead care plan meetings, ensure timely submissions, and educate staff on documentation requirements.

Qualifications

• Active NJ RN or LPN license.
• Experience with MDS in a skilled nursing facility.
• Strong knowledge of MDS 3.0, RAI guidelines, PDPM, and CMS regulations.
• Experience coordinating interdisciplinary care plans.
• Excellent organizational and communication skills.
• Ability to meet deadlines and manage multiple priorities.

Responsibilities

• Coordinate and oversee the facility's MDS assessment process per CMS guidelines.
• Ensure timely completion and submission of MDS assessments.
• Collaborate with nursing, rehabilitation, social services, dietary, and other team members.
• Lead interdisciplinary care plan meetings.
• Monitor assessment schedules, care plans, and documentation for accuracy.
• Utilize the RAI Manual, MDS 3.0, and PDPM guidelines.
• Identify opportunities to improve clinical outcomes and reimbursement accuracy.
• Maintain compliance and participate in audits and surveys.
• Provide guidance and education to staff on MDS documentation and assessment requirements.
