VIERA HEALTH & REHAB CENTER in Viera, FL is seeking an RN to serve as MDS Coordinator. You will manage the RAI process, complete MDS assessments, and support care planning, reimbursement, and regulatory compliance.

Collaborate with an interdisciplinary team to ensure accurate resident assessments, timely submissions, quality outcomes, and successful PDPM reimbursements. Prior MDS 3.0 experience and EMR proficiency required.

Qualifications

• Current Florida RN or LPN license.
• Minimum 1–2 years of skilled nursing or long-term care experience.
• Experience with MDS 3.0, RAI process, and Medicare/Medicaid regulations.

Responsabilités

• Coordinate and complete MDS assessments per CMS/Medicare/Medicaid/Florida regulations.
• Maintain MDS assessment schedules and ensure timely OBRA/PPS assessments.
• Develop, implement, and update individualized care plans with interdisciplinary teams.
