Job summary

Viera Health & Rehab Center in Florida is seeking an experienced MDS Coordinator (RN or LPN) to lead the RAI process, perform MDS assessments, and ensure compliant care planning and reimbursement documentation. You will collaborate with an interdisciplinary team to optimize resident outcomes while maintaining regulatory standards.

Ideal candidates will have MDS 3.0/RAI knowledge, PDPM experience, EMR proficiency, and strong attention to detail.

Qualifications

• Must hold an active Florida RN or LPN license.
• Experience with MDS 3.0, RAI process, and Medicare/Medicaid regulations.
• Knowledge of PDPM reimbursement methodology.
• Proficiency with Electronic Medical Records (EMR).

Responsibilities

• Coordinate and complete all MDS assessments per CMS, Medicare/Medicaid, and Florida regulations.
• Maintain MDS assessment schedules and timely OBRA/PPS assessments.
• Develop and update individualized resident care plans and coordinate interdisciplinary meetings.
• Ensure documentation supports Medicare/Medicaid reimbursement and regulatory compliance.
• Participate in audits, QA activities, and regulatory surveys; assist with surveys as needed.
• Educate staff on MDS documentation and regulatory changes.
