Job summary
Viera Del Mar Health and Rehabilitation Center is seeking an RN/LPN MDS Coordinator to lead resident assessments and care planning. You will collaborate with an interdisciplinary team to ensure regulatory compliance and optimize reimbursement while providing compassionate, resident-centered care.

Qualifications
- Active RN or LPN license in the state of employment.
- Two+ years nursing experience in skilled nursing or long-term care.
- Experience with MDS 3.0 and care plan development preferred; training available.
Responsibilities
- Coordinate and oversee completion of resident assessments (MDS 3.0) for timely submissions.
- Develop individualized care plans that drive quality outcomes and compliance.
- Monitor Medicare and Medicaid requirements and initiate coverage or notifications.
- Support nursing staff development and ensure optimal care delivery.
- Collaborate with leadership to maximize resident care reimbursement and meet operational goals.
