Job summary

University Park Care Center is seeking an RN MDS Coordinator to lead the completion and submission of MDS Assessments, CCAs, and Care Plans in alignment with CMS RAI Manual regulations. This role reports to the DON and collaborates with an interdisciplinary team to ensure compliance and quality care.

The ideal candidate holds an active RN license, has at least 2 years of nursing experience (geriatric preferred), and is CPR certified.

Qualifications

• RN license in good standing with the state
• 2 years of nursing experience, geriatric preferred
• CPR certification at hire or during orientation

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations
• Report changes in patient condition identified by MDS to the DON
• Educate direct care staff on CMS RAI Manual updates that affect documentation
• Review Interdisciplinary Comprehensive Care Plan
• Review CMS Reports for late assessments and improve processes
• Attend PPS, Triple Check, and other meetings as requested
• Perform staff nurse duties as needed
• Provide excellent customer service to patients
• Assist in patient evacuation
• Maintain attendance and professional conduct
• Follow company policies and compliance training
• Communicate effectively on an interdisciplinary team
