Job summary

Community Medical Centers is seeking an RN MDS Supervisor to oversee MDS assessments for all residents in our facility and ensure accurate documentation in the EHR. You will lead MDS nurses and coordinators, participate in interdisciplinary care planning, and support DON with audits and quality measures.

The role requires CA RN licensure, BLS, and at least 3 years of long-term care experience. A Bachelor's in Nursing is preferred, with training in MDS assessments essential.

Qualifications

• Education: High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved IEP Certificate required.
• Completion of MDS Assessment training class required.
• Experience: 3 years of long term care experience required.
• RN - Current and valid Registered Nurse license to work within the state of California required.
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association required.

Responsibilities

• Ensure accurate and timely completion and transmission of MDS assessments for all residents in facility.
• Oversee the MDS nurses/coordinators to ensure residents are properly assessed and documented in the EHR.
• Lead care plan meetings and participate as an active member of the interdisciplinary team.
• Collaborate with the DON and Director to provide information on residents, audits, and quality measures.

The RN Minimum Data Set (MDS) Supervisor is responsible for the accurate and timely completion and transmission of MDS assessments for all residents in facility. This role oversees the MDS nurses/coordinators to ensure residents are properly assessed and that this information is accurately documented in their electronic health records (EHR). The Supervisor participates as an active member of the interdisciplinary team and leads care plan meetings. Collaborates with the DON and Director to provide information on facility residents, including audits, areas of opportunity for care improvement and care conferences, as well as quality measures.

• Education: High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
• Completion of MDS Assessment training class required
• Bachelor's Degree in Nursing preferred
• Experience: 3 years of long term care experience required
• Experience in completion of MDS assessments required
• Licenses and Certifications: RN - Current and valid Registered Nurse license to work within the state of California required
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
