Qualifications

• Must hold a current RN license in Indiana.
• Working knowledge of the MDS 3.0 (current version).

Responsabilités

• Conduct and coordinate MDS assessments, develop individualized care plans, and ensure timely updates for significant changes in residents' conditions.
• Ensure accurate completion and submission of MDS assessments in accordance with Medicare/Medicaid guidelines and state/federal regulations.
• Facilitate care plan meetings, maintain effective communication across departments, and support continuous improvements in resident care outcomes.
