Job summary

Garden Terrace Alzheimer's Center of Excellence (Overland Park) is seeking an RN MDS Coordinator to manage and submit accurate CMS MDS assessments, CCAs, and care plans in accordance with CMS RAI Manual Regulations and Life Care standards.

The ideal candidate holds an RN license, an Associate’s or Bachelor’s in nursing, at least two years of geriatric nursing experience, CPR certification, and can coordinate interdisciplinary teams while ensuring compliance and excellent patient care.

Qualifications

• Associate’s or bachelor’s degree in nursing from an accredited college or university.
• Current RN license in good standing.
• Two (2) years’ nursing experience; geriatric nursing preferred.
• CPR certification required (upon hire or during orientation).

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI Manual updates affecting documentation.
• Review Interdisciplinary Comprehensive Care Plan and Final Validation Reports for CMS QIES submission.
