Job summary

Life Care Centers of America seeks an RN MDS Coordinator to oversee interdisciplinary MDS Assessments, CCAs and Care Plans in line with CMS RAI Manual Regulations. You will ensure timely submissions and coordinate with the DON on changes in patient conditions.

Ideal candidates hold an active RN license, have 2+ years of nursing experience (geriatrics preferred) and CPR certification, and will educate staff on CMS updates while supporting a compliant post-acute care environment.

Qualifications

• Associate's or bachelor's degree in nursing from an accredited college or university.
• Active RN license in good standing.
• Two years' nursing experience; geriatric nursing preferred.
• CPR certification at hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI Manual updates affecting documentation.
• Review Interdisciplinary Comprehensive Care Plan and Final Validation Reports.
• Ensure CMS QIES system readiness before billing and notify when not ready to bill.
• Attend PPS/Triple Check meetings and participate in other required meetings.
• Perform staff nurse functions as needed and maintain excellent patient service.
