Job summary

Life Care Centers of America in Columbus, OH seeks an experienced RN MDS Coordinator to manage interdisciplinary MDS Assessments, CCAs and Care Plans in full compliance with CMS RAI regulations. The role requires an active RN license, at least two years of nursing experience (geriatric preferred), and CRNC and CPR certification.

You will educate staff, review plans, and participate in daily PPS meetings to ensure accurate documentation and timely billing.

Qualifications

Active RN license in good standing.
Two years of nursing experience, geriatric preferred.
CRNC Certification (Clinical Compliance) and CPR certification on hire.

Responsibilities

Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
Report changes in a patient''s condition identified by the MDS Assessment to the DON.
Educate direct care staff about updates or changes to CMS RAI Manual or SNF regulations.
Assist with review of Interdisciplinary Comprehensive Care Plan.
Review CMS Reports to identify late assessments and develop prevention processes.
Attend Daily PPS Meeting and Monthly Triple Check, and other requested meetings.
Perform functions of a staff nurse as required.
Maintain patient rights and safety; demonstrate integrity and professionalism.
