Job summary

Regalcare in Wakefield, MA is seeking an experienced MDS Coordinator to join our skilled nursing and rehabilitation center. The ideal candidate is a licensed RN or LPN who can complete MDS assessments accurately and on time while ensuring regulatory compliance.

You will collaborate with an interdisciplinary team to update care plans, monitor resident progress, and participate in care plan meetings and quality assurance programs.

Qualifications

• Current RN or LPN license in Massachusetts (required).
• Minimum 1 year of MDS experience in a skilled nursing facility preferred.
• Strong knowledge of RAI process, PDPM, and Medicare/Medicaid guidelines.
• Excellent attention to detail, communication, and organizational skills.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Complete and coordinate MDS assessments in accordance with state and federal guidelines.
• Ensure accurate coding and documentation to support reimbursement and quality measures.
• Collaborate with interdisciplinary team members to develop and update care plans.
• Monitor resident progress and maintain regulatory compliance.
• Participate in care plan meetings and quality assurance programs.
