Job summary
Complete Care in New Jersey is seeking a Registered Nurse-RN-MDS Coordinator to lead accurate MDS assessments and personalized care plans. You will collaborate with an interdisciplinary team to ensure quality resident care, regulatory compliance, and appropriate reimbursement.
The ideal candidate holds an Associate's or Bachelor's in Nursing, an active NJ RN license, and prior long-term care or MDS experience.
Qualifications
- Associate's or Bachelor's Degree in Nursing.
- Current, unrestricted New Jersey RN license.
- Long-term care experience preferred.
- RN-MDS Coordinator or MDS assessment experience in skilled nursing preferred.
- Strong clinical assessment, communication, and organizational skills.
- Knowledge of federal and state MDS regulations.
- Ability to work collaboratively with interdisciplinary teams.
Responsibilities
- Coordinate and complete accurate MDS assessments per guidelines.
- Develop individualized resident care plans with interdisciplinary teams.
- Ensure documentation supports quality care, compliance, and reimbursement.
- Monitor MDS schedules and update documentation as conditions change.
- Participate in care plan meetings and contribute clinical expertise.
- Educate and support clinical staff on MDS processes and updates.
- Work with leadership to maintain compliance and drive quality improvement.
- Help Complete Care deliver resident-centered care.
